Fading Forms

I am looking for a code that I can use to fade in/out forms on load/close. I have searched all over this Forum and found a link to a zipped sample (HERE) and seems to be dead.

If someone could post the zipped sample db, I would greatly appreciate it. Please note, due to company internet policy, please do not hyper link to an offsite site. It must me associated with this forum. If it is hyperlinked, I will not be able to access the file.

OK, I am able to get the form to fade in, now how do I make it so that it fades out and goes to the main menu?

I am using the frmPopup form scrips to make this happen.
