Form not working properly

I have the form given in the database in attachemtn when i enter a date and click the first button i get strange dialog box that i should enter a parameter. Could someone tell me how to remove this functionality?

would that parameter be the second date? I.E.: the ending date? I notice that you have two boxes, what appear to be a start date and end date, however not speaking German... Now if you could translate that into Spanish...
