Having a form with the a set of fields already entered (1 Viewer)

I am trying to set up a database with a set of records that will be entered. The main thing is that I want a set of 18 names that will recur each time I open the form. Then at the time I open the form I want to be able to enter a date that will automatically be entered next to each name. Is this possible??
