Help Creating database

Would like help on creating an access database for my company which will record items delivered to our customers. I am attaching a report in pdf format that we want to print out.

The MAKE Field(Product’s Company name) field will have items like: Epson, smartpower, 3com , , and so on.

The MODEL Field will have the model of different product for one company.

The Description filed will have what that item is (if it matches the Product make and Model this should come in automatically)

The Serial# filed will be serial of that product. (No Duplicates)

For example:

MAKE | Mode | Description | Serial#

EPSON | TM-88 | Receipt Printer | J7887878
EPSON | TM-U200 | Kitchen Printer | F3489304
SMARTPOWER | LC80 | line condition 80watts | 56409090
SMARTPOWER | LC180 | Line Condition 180wa | 59i09090

And so on.

Want to fit about 40 items per page.

I know I need some sort of sub-form with probably 2 tables but don’t know how to proceed.

If any one has a sample database that I can use as a reference will be helpful.
