Help! how do i create a user friendly form to filter contacts??

Hi

I am a little lost. Not quite sure if this is done by query, pivottable or even report.

In my contacts database i have around 20 yes/no fields for publications contacts wish to receive. I need to create a form which allows a user to select a publication(s) and then show the contacts whom want these mailout(s). The idea will be to take these filtered/queried contacts and immediately transfer there email addresses to outlook. but one step at i time, so would appreaciate any advice on the best way to approach producing a form which will collect this data! help.

This is not good! You need to get this information into one yes/no field in a separate table.

do you mean give each publication its own record and link the tables one to many?

How would this be searched/queried through a form? would i be able to se the criteria for a query by linking through the form?
