HELP!!! I have no idea how to make this form!

Hey Guys,
I have to make this form in Microsoft Access for work asap and am unsure how to get the bottom section of the form and database setup.
I have made simple databases before like the top section, but i don't know how to get the layout and table to work correctly for the bottom section with 'Geotechnical, Acid Sulfate Soil and Contamination.

I have developed the top section where it says Source Info but am blanked at how to do the rest.

The design of the form at the moment is in Microsoft Excel but if i could make it look as close to this as possible, the better!

Please help!

I suppose you've one tables for each of the data "Master data" and 'Geotechnical, Acid Sulfate Soil and Contamination".
Then use subforms linked to the main form using the "Link Master Fields" and "Link Child Fields".
Else post sample data in an Excel sheet or MS-Access database.
