Help with subforms

Hi, I'm a newbie to access and I'm stuck in trying to work out how to open another form if the customer selects a certain tickbox. I only have one table with all the fields - date, referral, promotion, administrative, agency, norelevance, natureofenquiry, referredto, firstname, lastname, address, agencyname, etc..
referral, promotion, administrative, agency, norelevance are yes/no boxes. If the customer selects referral I would like another form to open or appear for them to put in the natureofenquiry, Refferedto, firstname, lastname, address, etc..
If they select agency I want another form to open for them to put in the agencyname.
If they select norelevance then that is all they need to enter. Please see file attached. I would really appreciate any help as I've been googling all day.

Use the Microsoft Access SwitchBoard Manager.

Menu: Tools | Database Utilities | SwitchBoard Manager

Play with it. You will see how easy it can be.
