How to create a survey?

Hello,

I haven't used Access for a very long time now and I was wondering if it would be possible to get some help on how to create a survey.

I am using Access 2003; but it says that I am using Access 2000 file format.

I have to create a questionnaire that will be given to people to get feedback on.

I need to have the layout of the questionnaire like this:

Name (Optional)

Set



Please give a score to each of the following statements:
5 = strongly agree
4 = agree
3 = neither agree nor disagree
2 = disagree
1 = strongly disagree

  • I enjoy Modern Foreign Languages.
  • I am making good progress in Modern Foreign Languages.
  • I understand clearly what I need to do in order to progress further.
  • OLL or homework is set regularly.
  • OLL or Homework is of the right standard for me.
  • Assessments are too difficult.
  • Assessments are too easy.
  • We have too many assessments.
  • I would prefer shorter assessments on a regular basis.
  • We have enough MFL lessons in ICT rooms.
  • I enjoy ICT based lessons.
  • I learn more in ICT lessons than other lessons.
  • I enjoy working from the textbook.
  • My teacher is well prepared for lessons.
  • There is lots of variety in my lessons.
  • Behaviour is very good in my lessons.
  • I think MFLs will be useful to me in the future.
  • I feel positive about continuing languages beyond Year 9.
I would be very greatful if I could get some help with this.

Search the forum for there are samples posted. If your company has sharepoint then I would create one in sharepoint instead of Access.

Also, I would change the default file format for your Access to always create new databases in the 2003 file format.
 
You could use an online survey generator like "surveymonkey"..
