Hope you will be able to find an answer to your question or to use that DB at all.

In order to make me understanding your request, quickly design a small DB with only the tables that you think that are necessary for this task, fill it with some data and upload this DB.
Use Access 2003 version and ZIP the DB before uploading.

PS: Use English for anything in this DB.

Unfortunately (for you) this is not how Access work.
Learn this
Database Normalization and Table Structures - Microsoft Access / VBA
then redesign your DB.

Also, take a closer look to my DB.

I created this form look and see I created , but I asked can create like this form with out relation , if you know tell me

It was your choice to design the DB (and the form) this way.

I can't debug something that is beyond my understanding.
Something that is, somehow, opposite to all I know in the DBs area.

Sorry for this, but really I can't help you.
