Question How to repeat and save recurring data

Hello and HELP!!!!
I need to design a simple clinical database that will allow me to collect and display recurring data. Example. Every year a patient comes into clinic for follow-up. They complete a fixed data set questionnaire, their vital statistics are recorded and any new clinical results recorded. I have created all the tables/Forms but i don't know how to automatically add a new event that repeats the same data. So far i have 5 excessive years worth of follow-up data. Sadly, I have had to make massive tables by copying the identical data with either a 1,2,3,,, to signify a different year. Extremely labour intensive and not very efficient :banghead:
Can anyone tell me how i can preform this operation!!!! I should mention that i would classify myself as an Intermediate Access user, so please, be gentle!

Yo don't need the same date in two tables (or more).
It means yor tables not normalized well.
Learn something about DATABASE NORMALIZATION.
 
It might help if you post a Pic of your relationships together with a story that points to the Relationships.

We could see by that what problems you are having.

I hear we will need to pack our own sandwiches for the Grand Final this year. Apparently there will be no Pies there. :rolleyes:

Thanks in advance for your willingness to help and apologies for not responding sooner- but I have given up trying to figure out how to paste/attach an image of relationship table.
Here’s the story:
Patients are recruited .
Each patient has a Medical History
One patient can have multiple surgeries.
One surgery will have one pathology report/results
One pathology can have multiple lab procedures
One pathology can have multiple research techniques
One patient can have multiple therapies (ie chemo/radio etc)
One patient can have multiple clinical follow visits
One patient will have one overall survival status (ie alive, dead, years post-op etc).

I have a total of 9 tables, none of which are linked! Except for the Patient table, all of my tables have the primary keys of all of the other tables as foreign keys. I have created many reference tables/combo boxes for such things as country of birth, surgical procedures and diagnosis etc. My problem is how do I add a new event to existing records? Patients may have additional surgeries, tests and every year they have a clinical review. All of this data has to be captured. I would like to see the clinical follow up information as I add to.

I did as you suggested but had a difficult time understanding. From what i glean from YouTube this seems to have something to do with standardizing data? I have created multiple reference tables for drop down fields for information that doesn't change ie Country, a diagnostic procedure etc. An example of what i need is this: To enter results for a surgical event #1, years later the same patient presents with another surgery #2. The dataset is exactly the same as the first, i just need to duplicate it and add it to the patient's record.
I have orders several self-help books and CD.:) Thanks!
 
Have you looked at the Northwind Database or any other Examples/Templates
