Can someone assist ...?

I need to incorporate a way of showing the date, time and UserID information of the first person to input data to our patient referral Form. In addition, it would also be great to include the UserID of the last person responsible for updating the data?

How do I go about this or is there a sample DB I could peruse.

I am sure MS Access has some built-in system parameters which could be utilised?

Forgot to mention, for the created date, time and by, look at the default value property of each of those fields.
The updated by, date and time is added to the Before Update of the form.
