Input mask date format am/pm no seconds

For the life of me I can't get an input mask to work for date/time. I want to store flight times like this:

Example: November 16, 2003 11:15 PM

What mask do I use? The following isn't working:

00/00/00\ 00:00\ >;LL;0;" "

I can't even directly enter the data into the table with no mask.

99/99/0000\ 09:00\ >LL;0;_
