You forgot the "frm" in the name of the form.

there are 2 forms, 1 with the "frm" in front and one without,

i want the name form to open the AIDSWalkRegistration form and subform to the proper recoed based on the name chosen

Yessir,

1) Get rid of the filter on the form with out the "frm":
[WalkerID] = 3
2) Do not call your date field "Date", that is a reserved
word in Access
3) Your form had the DataEntry property set to yes. Thus
It would only accept new records, not display old ones.
