Lookup Query? Help

I have the following scenario:

The company wants a system that will allow customers to go to an introductory form where they must enter the date and time they wish to go on the “Deer Valley Experience”.

How can I create a query to match the date and time entered to show only specific seats on a new form. Thank you.
