Main Form/Second Form

My question is say I am entering data for Mr. Johnson on my main form, and I need to go to another form or page to enter notes and comments, I want that second form to open up with Mr. Johnson's name and not at the being of the alphabet. (PatientID is my primary key on the main form)

The fields on the second form are as follows (PatientID, Admission Date, LastName, FirstName and Notes and Comments)

What do I need to do to accomplish this task?

Create a command button using the Wizard. In the first screen of the Wizard select "Form Operations" and then select "Open Form" in the right list box. Click the Next button. Select the form you want to open and then click Next. In the next screen select the radio button that says, "Open the form and find specific data to display". In the next screen select PatientID in both list boxes and then press the little button between the two list boxes. Click next and then select what you want your button to look like, then go to the last screen and name your button. You are finished and so am I. Good Luck!
