Make subform visible on form update

Hello! I'm not quite sure how to do this, but here's what I need:

I have a form with three tabs- a main form and two with three embedded subforms. After the initial record creation, if any of the tabs or subforms are updated, I want a subform to pop up requiring entry of initials and a change date.

So I would like to know,
1. How can I accomplish this? -or-
2. Is there a much simpler way to track who makes form changes that I'm missing?
