making a form to pull a query

I am a beginner...

I want to make a form that a user may select critera to pull a query. (I do not want the users to be able to enter the tables directly or do queries because we have lost data in the past this way).
I would also like it if I could make the query automatically convert to excel so that the users could play with the data there.
