the table tbQ4 has a form frmQ4 with yes/no question boxes regarding a patient history
for example
Fever y/n?
chills? y/n
muscle aches y/n?
general weakness y/n?

The report should include only the Yes answers for example

the patient has fever, muscle aches, general
weakness
