Solved Multi column display issues in Combo Box form

When I create a combo box display using 3 fields, I do not get the option of selecting which column value I need to capture. This screen simply does not show. I use Access 2016. I reinstalled Office thinking there is an issue with the installation, checked with Microsoft Support too. They have no clue

The various screens step by step are captured in a word file enclosed

Is there some configuration needed?

When I create a combo box display using 3 fields,
I think it has everything to do with the question since you're not creating a combo box in the first place. That is a control which can only be added to a form or report. You are creating a table level lookup field, which is something that many of us here would not use. Look at the first sentence of the second dialog page of the wizard.
