multi input form search ?

How do I do a form that I can enter
up to five items and then it will go
get me the information on those five
and display it on one form or popup
or something...(newb)

I have looked in the forums but, I am
still having trouble make one...heck...
I am even having trouble just getting
one to work...

I was hoping there was a way to type one
three, five or so items into a box and then
click a button and it would go find the info.
