Multiple Choice Questionnaire/Survey

I need to turn a paper based questionnaire into an electronic questionnaire but Im unsure how I should go about it. I need to keep it simple. My tutor says I should create a table of each section (Student, Delivery, Assessment etc). Any ideas? Thanks

Not very unique. The forum solved 2 of these last month. Just do a search on questionnaire or questionaire on this forum.

One of them had an excellent design already laid out. I can't think of a simpler way to solve the problem, unless you want to learn something.
 
Ok so I had a pop at it.

A table to store all the questions (tblQuestions)
A table to store all the answers (all "Strongly Agree, Agree, No Opinion, Disagree, Strongly Disagree" type answers)
A table to store student details (tblStudent)
A table to store the modules/subjects. The same questions need to be asked for 10 different modules.
And then a table for the response.

How am I looking? Thanks

It seems to look ok, as long as there is only one survey. If you need to manage multiple surveys, you need to add more.

Its the same survey that needs to be answered by the same user multiple times, once for each module/subject (eg science, English, maths etc)
