Name Checking when inputting

Not too sure which section this belongs in, and I know that you can help me with this little problem.

I am creating a database and there are two main things I am trying to resolve. Can you please help.

The database is going to hold peoples names, and these names will be allocated to a specific agency, and each agency has it own set monitoring chritera.
i.e. James Brown, ingtroduced by Go Go marketing. Go Go Marketing said that they would give James Brown, bread,milk,tea,coffe,soup.
Fred Jones, introduced by Jump away ltd. Jump away ltd says that they will give Fred (or any other person introduced by them) bread,soup,steak,tea

So... you have the idea, that each agency will offer different things (I`ll just use yes/no tick boxs for these. Ok now my problem

a) When I am inputting the persons name i.e James Brown, Is there any way the system will automatically show a list (in a popup window if necessary) of any names that match either surname/firstname. this is to stop more than one agency submitting the same person. This pop up box could then link to the full details of those people, so I can quicky check for double submissions.

b) The second part, well I have an idea about this but its very long winded, can you help. I have 12 agencies all with different monitoring tick boxes. What will be the best way to do this. Will I just create a table for the agencys and include ALL the monitoring boxs and just use the ones for the agency I need on the form. - This will mean I'll need to create 12 forms for monitoring- there must be a simplier way.

Any thoughts apprecaited.

Just one more thing, this might give you something else to think about.
When I enter the data, it will be baised on the following:-
Customer linked to Agency - linked to monitoring
So After I input the peroson name, and the name check has been done, I can choose an agency - IDeally, when I choose the agency - it will only pull the monitoring chritera that is accociated with that agency.
