Need Help With Database Needs

I need help with how to create what I need the database to do. For work, we need to make comments on how each person performs throughout the year. I need it so that each person has their own page and the supervisor has the ability to add comments with a new box for each comment. How do I do this?

You need to create a comment table that's basically a calander. Attach it to the emloyee ID, create a query with the employee ID as the criteria. You should be able to create a drop down on your page/form that's can access comments sorted ascending by date. You could go a step farther and have the query show only the latest comment on the page. If you need me to do a sample dbase in 2007 I can mock a quick one up for you let me know.

Starting something now for you. Its not going to be to advanced though.
 
Here is a sample dbase from access called Desktop Customer Service.

If you modify the fields tables and relationships to customers=employees
employees=managers you can create what you are looking for.in the form that opens is a drop down to the right for reports. Select the Cases Details Report in the zipped dbase and answer back if this is sort of what you mean but split per individual. Like I said this is a quick mock up that would need more work but it is relatively easy to do.
