I have a "Facility Site Assessment" database. There are 92 questions that must be answered for each assessment conducted. Answers are either Yes, No or N/A.

In my questions table, I have the Question number, question text, a Recommendation, Reference and Response. There is a prewritten Recommendation, reference and response for every question. The report needs to display all 92 questions and their answers (Yes, No, N/A), so the contact at the facility knows how they fared on all topics of the assessment. But ONLY if a question is answered "NO", then the recommendation, reference and response need to appear beneath that question, so they have instruction on how to resolve the problem. How do create a report that does that?

I appreciate any guidance!!!

I think that the best place to apprach this is in a query.
In the response field for each response place NO in the criteria.
Then use the query to create the report.

Still wondering?

Thank you for the reply. But if I set the criteria to No, then it will only display the questions that were answered with "No". I need to display all 92 questions on the report, and their respective answers (Yes, No, or NA), but IF the answer is NO, then I need it to also display additional information (recommendation and resource information for Supervisors to correct that issue). I don't know how to have it display the content of a text box only if the value of another text box is "No".

bambam,

look into conditional formating. It's available in reports for Access versions greater than '97. It should get you what you want.

Thank you for your suggestion. I ended up making three different queries (each to return the questions answered: No, Yes or N/A). And then I made 3 sub reports. It might not have been the best way to do it, but it works.
