I've been asked to make a database which does the same job as the attached excel document.

Someone on a machine enters the details of the job they've just done like the length of the metal and the alloy it was and it should all get stored.

At the top of the page, there's the information which identifies when the jobs were done and by whom - the date, shift (morning/afternoon/night) and clock no. (each employee has their own clock no.)

I have no idea how to structure my tables though - all the guides I've managed to find on relationships are either way too simple or way over my head.. I can't find anything which properly explains how I can make entries in one table - the details of the job - "belong" to a particular date, shift and clock number on another table..

I could make one single table with the following fields like this:
Billet No.; Bore id; Alloy; Length; Speed; Feed; Time on; Time Off; Bar Change; TimeLost; Time & Reason; Breakdowns; Comments; Machine No.; Date; Shift; Clock No.

but I thought that loads of the information would be recurring and the whole point of databases was to eliminate repeating information...

How would you go about converting this hand filled form into a database? :s
