Pre Populated Report Fields

Hi

Can someone please help me - what I would like to do is have some fields on a report pre populated by some details a user inputs on a form.

The form has a combo box for Religion.

On the report I have a list of all religions with a tick box next to each of them.

I would like the selected religion to appear ticked if possible on the report. Unfortuntely I have to keep both the form and the report in the existing format.
