Query Value Question

I have a bit of code I have tried to modify to give me what I want but I need help with last little bit.

I want the RN and A as the rows and I want the departments as the columns.

I have a problem with the rows because it only shows me one RN and one A their should be 6 RN and 4 A. Not sure how to fix this.

I do not want the time in the value at all but when I try to take it out I break it. So I need to know the proper syntax to only show the full-name field.

My two problems are:

1. only show name as value
2. show all RNs and As not just one
