Records are being changed/deleted!

I have a Teams table that seems to have an intermittent problem. Records in the table randomly seem to be lost or sometimes data within the record gets switched around with other records! For example, yesterday I had two certain records: Team 9a, Team 9b. When I went into this table, there were two records for team 9a and no records for team 9b. Thinking some user went into the table and messed around (which they shouldn't be doing, users should only be using their Data Entry Forms that I've written and staying away from tables), I corrected the problem and dismissed it. Today, I went into the table, Team 11b was in the record where Team 2a should have been, 2a was gone completely and 11b was also in the original record that it should have been in. The only thing else I can think to say is that this table feeds Combo Boxes on two separate data entry forms. Also, the db is on a shared drive and has a maximum 4 users at any given time. I have decided to leave the file unsplit as tests with an FE and BE resulted in it being ridiculously slow (even Select Queries took over a minute to load). Any thoughts?

Well, they're not "Find Record" combo boxes. The combo boxes take the Primary Key Autonum from the Teams table and are bound to foreign keys of different Site Info tables. They obviously will change the foreign key tables, but the Teams table should remain unaltered. This project has been in use for about three weeks now and is just now beginning to get this error.
