Hi,

I have this form set up for users to do report.

It's a pop up form with a couple of fields and two buttons. One is for opening the report and the other closing the report.

Sometimes users may not remember what fields they chose after the report has been opened.

Is there any way to somehow record either on the pop-up form or on the report the fields they have chosen to report on?

It's almost like a reminder that kind of thing. Maybe I should use unbound labels or textboxes?

any thought/comments much appreciated.

Try this;
In the Report (on the top), put the unbound textbox(es), in these
textboxes (in Control Source properties) put
=[Forms]![FormName]![FieldName]
and try.

Don't know why Mstef had to add to what I had posted but for the record Forms and Reports have Controls, Tables and Queries have Fields.
