Search Form Design

I'm pretty new at this,so could someone help me do the following:

make a search for that has


a text box for the user to type what they want

a list box that does a wild card search for results (clicking results opens form with the selected record)

a combo box that allows the user to change the field that is being searched

See the following thread. Download the sample db in the last post. It should give you an examp-le of exactly what you are looking for.

Let me know if you have any problems with it.
