I open a form and attempt to add a record to its subform...

I am getting the message:

"the changes you requested have not been successful because they would create dublicate values in the primary key or relationship"

What cause this?

Check your constraints. Sounds like you have a unique key that is being duplicated by entering that data.

can this happen if you alter the 'one' part on a 'one to many' relationship?

Most likely. If that's what you did, you need to change the linkage in the subform.
