Trying to limit drop-down list

Hiya, I have a drop down menu in my form for names, and although you can pick from it...everytime I type in a name instead of just picking it iti goes into the drop down twice, so now i have 6 or 7 duplicates of the same name!!!
I could just put into a fixed list but sometimes a new name the user has to put in a name not on the list!!!
What am i doing wrong.....

Did as your link suggested, but still duplicates are appearing on my drop-down menu!!!
Do you have any other suggestions??
