LinaRose, I built a very quick example in Access 2010 so you can see how it can be done. Open the form and select a job number, then a month, then click the Report button.
I know it's not fancy, but you should be able to decompose what I've done, paying especial attention to the comboboxes on the form and the parameters in the query. Hope this helps!
