Using a form to query a table with multiple criteria and multi select list boxes.

Hello all,

I am trying to create a search form which allows the user to multi search and return the result as a query output

Using multi value list boxes combined with combo boxes with the ability to not select criteria from every box

I've made an attempt but it isn't working very well. I've spent hours on it and am getting into a twist

I've attached a very simple version. Could someone possibly have a look for me..
