I changed the form from before (link below). It seems there were some issues with it.

The sample db also includes a time-out function along w/ some validation requirements. It also has spell-check and calendar functions. Please respond if there are any mistakes or problems.
Access 2003--Some versions will give you errors when closing the Assessment Form but other than that it works well.
The timer function is set to one minute of inactivity.
