Add Prefix to Index according type selected

hi! i have a form for our reference number and we need to add prefix according to the type of letter. here is the sample :

gitnangsilangan.com/eyeseven/form.jpg

for example i choose the "letter" in my type field, the the letter "L" will be added to the auto number field (EDSCO/1147/L). and if i choose Quotation "Q" will be added (EDSCO/1147/Q)
