Assesment Database for teachers......need help with table design please

Hi all

first, thanks for taking the time to read this....

Heres the senario, Im a ICT technician at my school and my PE teacher in putting together an assessment grid to monitor pupil progress. I have attached the word file that he gave me to work from..............

i was fine with the initial format, but im so crap with databases i kinda lost with how to set up the tables.........heres how it should be

Each Student is working at a "Level" and these are, P4, P5, P6, P7, P8, 1

The sheet i have attached is P4, just to show an example

No within each level, there are numerous statements divided into 5 categories. the statements vary for each level, so therefore i decided that each level should have its own table as a student will have a 1:1 relationship with each level.........

But heres where i get stuck, i have started the database and have put each statement as a column in the level table, each statement is assessed and has a level assigned to it according to:

1: what level the student is working at, it will always be between 0-3 ( the attached word document will explain this better that i am)

2: which area the student is demonstrating as meeting that statment.. this is shown on the word doc (" OO, EXP, ID, AR, PML, EX")

No i do not know how to design the tables to show points 1 and 2 in relation to each statement accross all the levels.

I hope ive made sense.......if any furthur clarification is needed please let me know...........i know my explanation skills are dire.....

Just very quickly before I go home, you should make each of the highlighted sections a database and the lists below each the fields that the pupil will / should attain. Label the attainments as per your list and collect the values of 00, EXP etc set to between 0 and 2 (0 being where no attainment has been achieved).
