booking in form

Hello all
i am trying to create a booking in form for a small company.
basically i need a form that has fields like name, address, telephone, items left etc
i then need this to have a print button so i can print the form.
i have tried editing the office templates like the "customer service" however i have had no luck
can someone point me in a direction of a booking in form template?
