Calculating Percentage from Form

Hopefully a simple question. I have attached a sample of the database with the question. I would like a percentage of compliance auto calculated from the fields completed on a form. Please review and give me your thoughts. Thanks in advance.

ahh slight hard than i imagined - however if you trawl through the samples - this ahs been answered within the past 6 weeks -

its all about counting the responeses and making it variable

Trying to find the examples and haven't had any luck. Does anyone know where to find it?
