Checkbox search form?

Hi all, hoefully someone will be able to help me with this. My idea basically is that my form will have 3 check boxes on it a command button and a combo box for results. What i want to achieve is that when the user fills the relevent check boxes the hits the command button the combo box will fill in automaticaly with the results (names) dependent on which check boxes were filled in.

Any help on this matter would be great I havesearched the net and cant find and tuturials ect for this.

To be a bit clearer heres a example of my table i have fields for memberName, memberID, memberAdd, memberPhone, which are all text fields then i have yes or no fields for activity1, activity2, activity3. On the form im attempting to do i have 3 checkboxes act1, act2, act3, a Command button for the search and a combo box to display the memberName after search dependent on whichvariations of check boxes are ticked. So i can search for which member has done activity's 1+3 but no 2 for instance.?

Iv managed to find out how to the check boxes using else if statments but could anyone help me on how i would then get the memberName or Names to display in a combo or text box?

Must it be checkboxes? I have a database that includes a fairly powerful search facility that gives my users text boxes where they can enter all or part of a particular field (number, text, date, whatever), and then click a command button and have a list populate depending upon every match and/or partial match.
