I need my report to display all the test results of the patient including ones that are not done.

patient birben has just a sonogram test. but has not done any other test. (HSV 1/2 IGM, ETC, ETC)
I need the report to be generated like this
TEST DONE?
HSv 1/2 IGM NO
HSV 1/2 IGG NO
SIGN HIV CONSENT NO
SONOGRAM YES
etc etc all the test should be layed out on the report whether test was done or not
