Combine a record from few row into one row

I have the table like this:

Place code Name
12 Mania
13 Donia
12 Kamal
12 Manisa
13 Pato

I want to make new table like this:

Place code Name
12 Mania, Kamal and Manisa
13 Donia and Pato

Can someone help me?

You should be able to modify this to add the "and" to the last record:
