Combo box disappears from Form (1 Viewer)

Hello All,

I have a Form with 3 combo boxes. The option selected is referenced in the query I am adding under criteria Like [Forms]![TestReportWPForm]![CmbPrgArea] to pull data based on the selection in the form. When I add the query name in the record source form, the Combo box disappears.
I have a report also based on the same query to display records based on the combo box selection.

I have another database with exactly same setup and its working fine. I do not understand what the issue is...please help.

Suggest you provide db for analysis. Follow instructions at bottom of my post.
