DATA entry in Word Doc

What I would like to do:-
Create a document that has fields that need populating by a user. The document will have already been created with fields left blank for the user to fill out. The user can from a filed/form complete application information ie Name, Address, Date etc and once complete the data entered by the user will then be transposed onto the now completed document.

Can you please give me some ideas on how best to achieve this.

Display the forms toolbar. Double click on the abl button to insert a text field at the area you want it placed. You can double click on the text field to add help and functionality.

Save the document as a template and when the user opens it they can press the F11 key to move through the fields, completing them as they go.

Hi I have done this but double clicked on the text field and don't understand how to get data from my access file?
