Data Entry on Form

I have become frustrated.
attached is my database. It is an inventory
The frmNewEquipment should be used to add new records to the inventory.
I have it set up from a query, with the combo boxes set to populate text boxes (such as point of contact info from the location of the equipment). I also have a combo box for the type of equipment (cpu monitor etc). When I try to add a new entry, I get a error message about linked tables, and I am not allowed to put any data in the make, model, serial number fields. can someone help??

I've changed you form a little bit. It no longer runs from a query and I've added a cmd button, which updates the tables.
