Design Question

I'm setting up a form for a friend & would like to know the best way of getting it done. He has surveys that allow for multiple answers for each of 14 fields. There could be different combinations of 5 answers. Please see example:

Field = Current Occ Proj - Survey Response = Internet, Newspaper

Field = Ind Empl Data - Survey Response = Internet, Special Studies

Field = Occ Wages - Survey Response = Newspaper, Call Someone, Newsletter

Altough I'm not a big fan of using them, if I were doing a quickie, I would just use the 'Lookup' tab in table design to set up these flds.

Think this would work?

Im not too fond of lookup either but I went ahead and designed it that way. Thanks!
