Excel 2007 quick access/custom tool bar questions

My Excel 2003 custom toolbar appears in Excel 2007 via Add Ins but if the user clicks one of the other tabs the custom toolbar is hidden and the user has to click Add Ins again.

How do I keep the Custom tool bar visible all the time that the workbook is open?

How can I add custom buttons to the the quick access bar using code so that they are only available when the associated workbook is open?

How do I change the picture on a button on the quick access tool bar to one I have created. I seem only to be able to change the picture to one
supplied by Microsoft?

It is possible to place the macros to buttons on the quick access toolbar and have these buttons only avilable when the asscociated workbooks is and I'm not sure about this, open or has focus.
not sure it is possible to change the picture on the button face to one of my choice, may be only to one of a pre-selected group. That's pre-selectd by Microsoft!
