Export Data to interactive pdf (1 Viewer)

Is there a way to export my table or form fields to a specific pdf file on my system that has interactive fields (its pdf that has several fields that can be filled in on the computer).

So lets say I would like the last name to go into the Q1_Last_Name filed on the pdf, the first name should go into Q2_Your_first_name field on the pdf, and so on.

The short answer is yes, it can be done. The longer answer is - we had done it but it was a pain in the backside and, while I know it can be done (we used the ActiveX control from Stephen Lebans but it didn't work for us in 2007), I was fortunate enough to have that functionality killed just after I started working here.

Not necessarily but, if possible it would be better to have a Report built to look like the Interactive PDF that you can run and save that out to PDF instead.
