Form Selection for Query Criteria

Guys I want the user to be able to select the month and year for invoice statements they wish to view via a form as opposed standard query input parameter ialog. On selecting 'cmdInvoice' from the main customer menu I want the 'frmLookUp' to open (it does) and the user to select the month and year from the combo's (they can). After this I run into difficulties!! I have had to butcher the DB to get the size down for the forum, but hopefully you get the picture of what I am trying to acheive. If any one can help (sample DB, another thread etc.....) I would appreciate the help!!

can u convert this to access 97 format.

Access 97

97 Format as requested. Hold down the shift key to go to DB Window.
Apologise for the poor DB, really had to strip it to get it on the forum.
