I need a form in the laboratory database which has separate groups of tests like hematology, chemistry, serology and each group has its respective tests, when I select one of the groups, the tests in them are selected.

You seem to be asking us to build an entire application for you for free. You've posted two links to similar products that are not free. You should purchase which ever one makes the most sense for your needs.
